Duties: To work as a Band 5 Physiotherapist participating as a member of the Home First team which includes the front door, managing a clinical caseload with the support and guidance of Senior Physiotherapists. The post holder will provide physiotherapy assessment, intervention, rehabilitation and evaluation for patients within acute hospital and community settings, supporting safe discharge planning. The post holder will have no budgetary responsibility but may be expected to supervise therapy assistants on a day-to-day basis. This role offers physiotherapy assessments and interventions to Service users. They will have the skills to maximise independence and to work flexibly to allow people to remain in their own homes by delivering specialised rehabilitation programmes.
