Care That Starts
With You
Our Nurse-led complex care supports children, young people and adults at home, shaped around individual needs and goals.

Getting To Know You
Your journey with us begins with listening. One of our experienced Nurses will meet with you to assess your needs, understand your routines and explore what matters most to you. Together, with the other professionals who support you, we will discuss how you would like your care delivered and agree meaningful outcomes.
You will also meet your care manager, who will oversee the day-to-day delivery of your care and provide a familiar point of contact. We will build a core care team with the skills, training and assessed competencies to meet your needs, introducing them to you and your routines before care begins.

Engaging With Your Community
Your care will empower you to be an active member of your community. Whether this is working, enjoying activities, or spending time with family and friends, we plan care to support you.
For children and young people, this also includes participation in education, activities and family life, reflecting their needs as they grow. We work with each child and their family to understand what matters to them.
As goals or circumstances change, we review your plan together so that care continues to reflect your needs, choices and ambitions.
